With heavy hearts, we announce the death of JoAnn Forizs of North Liberty, Indiana, born in South Bend, Indiana, who passed away on September 17, 2023 at the age of 81. Leave a sympathy message to the family on the memorial page of JoAnn Forizs to pay them a last tribute.
She was predeceased by: her parents, Marshall Holycross and Nina M. Holycross (Feece); her husband John L. Forizs Jr.; her sons, Ricky and Kevin; and her siblings, Harry Holycross (Flo), Marshall Holycross (Kay), Linda Burns (Mick), Raymond Holycross and Kathy Holycross-Wing (Derle).
She is survived by: her children, Lori Hagedorn (Lee) of North Liberty, Jamie Kharga (Prabhat) of North Liberty, Jeff Schrader (Jennifer) of South Bend, Lisa Pham (Robert) of Rensselaer, Minh Nguyen (Yen) of Long Island and Ted Clark of Plymouth; her grandchildren, Kristy Brennan (Jerry), Eric Bueno, Libbi, Joe, Zack Schrader, Maya Kharga, Giang (Lynda), An Nguyen, Victoria Morris, Vivian, Violet Nguyen and Olyvia Clark; her great grandchildren, Mari, Matthew, Mason Brennan, Sophia, Isabella and Amelia Nguyen; and her sisters-in-law, Pam and Brenda Forizs. She is also survived by numerous nieces and nephews.
JoAnn's family wishes to thank the staff and doctors with the Hospice Foundation for their compassionate care given to their mother. Memorial contributions in lieu of flowers are requested to be made to the Center for Hospice 112 S. Center St., Plymouth, IN 46563.
