He was predeceased by: his parents, Philip Cook and Stella Cook; and his sister Debbe Bailey. He is survived by: his significant other Karen Ford of San Pierre, IN; his daughter Stephanie (Jason Dyjak) of Wanatah, IN; his granddaughters, Katie McCormick (Andrew) of LaCrosse, IN. and Sami Vance of LaCrosse, IN.; his siblings, Sandy Feely (John) of Speedway, IN and Jan Krulik (Tim); and his brother-in-law Jerry Bailey of North Judson, IN. He is also survived by several nieces and nephews.
