He is survived by: his wife Carole Blake (King); his sons, William Blake (Dawn) and Tom Blake; his brother Robert Blake (Eleanor); his grandchildren, Abbey Levenshus (Jonathan), Adam Blake (Lauren) and Tyler Blake (Melissa); and his great grandchildren, Rose, Hadley, Andrew, Beau, Grace, Henry Ozzy and Eli.
