He was predeceased by: his father Henry Cook; and his sister Sandy. He is survived by: his mother Beulah Cook; his wife Vicky; his siblings, David (Pam) and Beverly (Chris); his children, Amy (Jeff), Michael (Crystal), Mark (Alicia), Daryl (Nicole), Brandon and Kelly; and his great grandchildren, Josslynn (Brody), Elliana (Julien), Iris, Kylee (Eli), Draven (Liam), Presley (Alex) and Gracelynn.
