He is survived by: his children, Myles Cook of Sarasota, FL and Madyson Cook of New Castle; his grandson Carson Cook of Sarasota, FL; and his siblings, Barb Norton (Walter) of Crawfordsville, Mike Cook (Denise) of Lizton and Richard Cook of Greenfield. He is also survived by several nieces and nephews.
