He was predeceased by: his parents, Piotr Kulaga and Karolina Kulaga (Sawina); and his siblings, Stephen Kulaga, Alfreda Palka and Genevieve Hyla.

He is survived by: his wife Janet Kulaga of New Albany, IN; his children, James Kulaga of New Albany, IN, Kathleen Johnson (Perry) of Georgetown, IN and Jennifer Jaffe (Christopher) of Bloomington, IN; and his grandchildren, Matthew "Severin" Johnson of Georgetown, IN, Benjamin Jaffe of Bloomington, IN and Lorelei Jaffe of Bloomington, IN. He is also survived by several nieces, nephews, and their extended families.

In lieu of flowers, donations may be made to Gentiva Hospice Foundation or the American Legion.
