He is survived by: his wife Kathy Blake (Dotts) of Mitchell; his sons, Guy Aaron Blake and Jonathan David Blake (Stormi Lynn); his grandchildren, Chelsey, Zachary, Christian, Arianna and Ezekiel; his great grandchild Easton; and his brother Albert Blake (Janie).
