He is survived by: his wife Sandra Lewis of Mitchell; his children, Mark Lewis (Tony) of New York, Marty Lewis (Monica) of Indianapolis, Jason Lewis (Sylvia) of Seattle, Trevor Guthrie of Mitchell, Teesa Fitzpatrick (Chris) of Mitchell and Allison Hale (Chris) of Ferdinand; and his grandchildren, Andrew, Walt, Gwenna, Elizabeth, Katherine, Tristan, Grant, Tyler, Kyle, Darby and Dillon.

Due to Covid-19, guests are required to wear a mask.
