She was predeceased by: her parents, Kensey Cundiff and Eunice Van Slyke (Frye); and her siblings, Harry, Mary and Bunny.
She is survived by: her husband Buddy Fields of Owensburg; her children, Toby (Marilyn), Tammy (Gary) and Troy (Sandy); her grandchildren, Misty, Marcy, Jason, Tara, Tegan, Tayla and Tia; and her brothers, Roy, Clarence, Dean and Bub.
In lieu of flowers, the family requests donations be made to I. U. Health Hospice.
