He was predeceased by: his parents, Goldie Cook (White) and Floyd W. Cook; and his brother Bill Cook. He is survived by: his wife Sally Cook (Brown); his children, Cathy Lehman (Roger) of Sunman, Kent Cook (Gai) of New South Wales and Kevin Cook (Shelley) of Lawrenceburg, Indiana; his grandchildren, Emily Holler (Chad Holler), Sarah Lehman, Olivia Lehman, Mya Cook, Mason Cook, Alexandra Cook, Katirenya Cook, Isabella Cook, Hayden Cook and Hudson Cook; and his brother Tom Cook (Sally Cook) of Milan. He is also survived by sister-in-law.
His donation is and will help so many people. Memorial Donations may be made in his name to the Epilepsy Foundation, American Heart Association, or Juvenile Rheumatoid Arthritis Foundation.
