He was predeceased by: his parents, Vincent Herbert and Mary Jane Herbert (Kirchbaum); his wife Shirley Herbert (Moody); his son Thomas Herbert; and his siblings, Dale Herbert, Lydia Smith and Ginny Bedel.
He is survived by: his children, Robert Herbert (Lori) of Elizabeth City, Vernon Herbert (Brittany) of Milan, Tina Ashcraft (Randy) of Milan and Debbie Arthur (Albert) of Milan; and his siblings, Vincent Herbert of North Vernon and Donna Holman of Greensburg. He is also survived by 24 grandchildren; 57 great-grandchildren; and 18 great-great-grandchildren.
Memorials may be given in honor of David to the Milan Fire Department or Grace Baptist Church.
