He was predeceased by: his parents, Forest Maddy and Joan Maddy. He is survived by: his siblings, Faith Diane Maddy and Brian Lee Maddy (Krystin); his niece Kaylee Davis-Maddy (Sam Washburn); his nephew Ethan Brian Maddy; and his great-niece Eleanora Maddy-Washburn.
