She was predeceased by: her parents, Charles W. Burns and Inez G. Burns (Pearcy); her husband John Robert Cook; her son Robert D. Cook; and her brother Harold Burns.
She is survived by: her daughters, Rita Williams of Martinsville and Vickie Stephany of Martinsville; her brother John D. Burns of Indianapolis; and her grandchildren, William T. Cook (Londa), Michelle Terrell (Mike), Nikita Urban (Andrew), John T. Williams, Donna Rinehart (Nathan) and Michael Stephany (Jackie). She is also survived by 23 great grandchildren.
