He was predeceased by: his sister Carole Maxwell. He is survived by: his wife Betty Jo Maxwell (Hall) of Martinsville; his children, Dr. Coby Maxwell (Alison) of Zionsville and Cori Sparrow (Josh) of Martinsville; his sister Marlene Maxwell of Martinsville; his stepchildren, Grant Collinsworth (Susan) of Martinsville and Terri Ann Miller (Tim) of New Palestine, IN; his grandchildren, Hunter, Cole, Brock, Bryn Maxwell, Maddux and Hadley Sparrow; and his stepgrandchildren, Madeline, Grace, Graham, Terrin Collinsworth, Macy and Nicholas Miller.

Contributions may be made in lieu of flowers to one of Terry's favorite philanthropies, Martinsville Noon Lions Foundation, PO Box 1062, Martinsville, IN 46151.
