He is survived by: his parents, Donald Smith and Sherry Foxworthy Wallace; his spouse Camille Hasik; his children, Kimberly Rice (Daniel) and Michael Hasik; his grandchildren, Blake Cross, Dalen Ramie, Charlotte Rice, Tyler Rice, Hollie Kouns and Kenley Turner; his sister Anita Howard (Shawn); and his nephew Chris Howard. He is also survived by other nieces, nephews, and cousins.
