He was predeceased by: his parents, Louis O'Neal and Mamie Brooks O'Neal; his siblings, Michael O'Neal and Donna Wilson; and his half-brother Louis O'Neal.

He is survived by: his granddaughter Amy; his wife Sherry Lynn Kinman O'Neal of Madison, Indiana; his children, Israel O'Neal (Crystal) of Edinburgh, Indiana and Amy Fields of Madison, Indiana; his grandsons, Damon O'Neal, Ryan O'Neal (Angelique), Brady O'Neal and Walker Lee Lucas; his siblings, Pam Sturgeon of Plainfield, Patty McClure (Mark) of Madison, Indiana and Kevin O'Neal (Greg Jewell) of Louisville, Kentucky; and his half-sister Janet Hamilton of Louisville, Kentucky. He is also survived by several nieces, nephews, and other relatives.
