She was predeceased by: her parents, Elisha Moore and Marie Hays Moore; her son Billy Ray Caldwell; her former husband Jerry Caldwell; her husband Jess Jacobs; and her brothers, Maynard Moore, Wade Moore, Raymond Moore and Wayne Moore.
She is survived by: her children, Sandra Smith (Clinton Wiley) of Commiskey, Lorri Nicholson of Madison, Indiana and Anthony Wayne Caldwell (Barbara) of Madison, Indiana; her grandchildren, Christopher Brian Gilpin, Lisa Jeannette Gilpin, Steven Corey Gilpin, Anthony Caldwell, Traci Caldwell and Jase Shelley; her great grandchildren, Alexis Ann-Marie Lay, Alona Marie Delaney, Corey Neal Gilpin and Scarlett Caldwell; and her siblings, Sylvia Wampler (Randy) of Franklin, Indiana and Roy Moore of Morgantown, Kentucky. She is also survived by several nieces, nephews, and other relatives.
