He was predeceased by: his son Michael O'Toole; his brother James D. O'Toole Jr.; and his aunt Audrey Driggs. He is survived by: his wife Susan O'Toole; his son Martin O'Toole of Liberty; his husband Gena Waltz of Liberty; his grandchildren, Ryleigh Waltz of Colorado and Payton Waltz of Indiana; his cousins, Elizabeth Burt of Tucson and Patricia Burt Sullivan of Brattleboro; his sister-in-law Carol O'Toole of Downers Grove; his close friends, Ron Stolle and Carol Stolle; and also Mike Waltz of Liberty. He is also survived by one niece; one nephew; three great nieces; two great nephews.
