He is survived by: his sons, Terrance Kolodzik of Georgia, Thomas Kolodzik of Georgia and Tyler Kolodzik of Texas; his stepdaughter Shannon Smith of Laurel; his siblings, Donald Kolodzik of West Virginia and Wanda Bevis; and his grandchild Jordan Smith.
