He was predeceased by: his parents, Robert M. Sullivan and Edith C. Sullivan; his siblings, Robert "Bob" (Donna), George (Patsy) and Clara Spidel; and his nephew Bobby Sullivan.
He is survived by: his children, Terri Anliker (Ken) of Chalmers, IN, Michelle Evans (David) of Lafayette, IN and John T. Sullivan (Deborah); his grandchildren, Andrew Evans, Ian Klinker, Breanne Evans, Markie Sullivan, Kent Evans (Alexandra), Shane Sullivan and Tai Audi (Shadi); his great grandchildren, Brenden, Sailor, Jack, Piper and Lennon; his brother-in-law Jim Spidel; and also Joyce Anne Sullivan (Cooley).
In lieu of flowers, memorial contributions may be made to the Forty and Eight Nurse's Training Scholarship Program or The Underprivileged Children Fund.
