He was predeceased by: his parents, Edmond Mazur and Mary Lou Mazur. He is survived by: his wife Linda Polarek; his stepdaughters, Jennifer Hernandez (Juan), Connie Jaquess (Ron), Julie Weber (Mark) and Sandra Mitchell (Sean); his sister Sharleen Stout of Elkhart, IN; and his daughter Nichole Bishop of La Porte, IN.. He is also survived by 8 grandchildren.
