Forker (Shelley) of Sarasota, FL; his grandchildren, Liam Jonathan and Svea Lynn Forker of Sarasota, FL; his stepdaughter Madison Pavey of Ashville, NC; his brothers, Mark Forker (Chris) of Valparaiso, Dr. Thomas Forker (Eileen) of Williamsburg, PA, Joel Forker of Knoxville, TN and Neil Forker (Ana) of Payson, AZ; and his siblings-in-law, Debbie Mazar (Adrian), Donna Homann (David) of La Porte, Deidre Notto and Ron Caddo of La Porte. He is also survived by many nieces and nephews.
