He was predeceased by: his parents, John Kozlowski and Julia Kozlowski (Pataluch); his wife Dorothy B. Ciesielski; and his siblings, Sophie Gadzala, Stanley Kozlowski, Lottie Mikolajewski, Mary Skwiat, Adeline Levendowski and Leo Kozlowski.
He is survived by: his siblings, Aggie Craig of Michigan City and Chester Kozlowski of South Bend. He is also survived by many nieces and nephews.
