It is with great sadness that we announce the death of Hazel Mullins of La Porte, Indiana, born in Jackson, Kentucky, who passed away on September 19, 2022, at the age of 73, leaving to mourn family and friends. She was predeceased by: her parents, Earl Stacy and Patsy Stacy (Miller). She is survived by: her husband Farley Mullins; her daughters, Farlene Ann Prater (Booter) of Jackson, KY, Rebecca Rose Koenig (Tom) of South Barrington, IL and Elizabeth Michelle Mullins of La Porte; her siblings, Shelby Stacy of Jackson, KY, Thurman Stacy of Jackson, Clifford Stacy of Union Mills, IN, Earl C. Stacy of Jackson, Gladys Howard of La Porte, Rose Mullins of Jackson and Lesa Vallo of La Porte; her grandchildren, Emily Byrd, Taylor Koenig and Kayla Koenig; and her great grandchildren, Brooklynn Mullins, Parker Mullins, Eve Mullins and Jayson Byrd.
