Hazel R. Levendoski Obituary
We are sad to announce that on July 8, 2023, at the age of 72, Hazel R. Levendoski of La Porte, Indiana passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parents, Howard Smalley and Leota Smalley (Cochran); and her son Eric Jon Levendoski. She is survived by: her husband Jon; her daughters, Margaret Lummis, Cheryl Warner, Kimberly Matthews and Carrie Howard; her grandchildren, Michael, Ronnie, Jessie, Little Jon, Gabriel, Callie and Corrine; her siblings, Donald Smalley (Sue), Jerry Smalley, Mercy Adams (Bill), Anne Hensley (Randy) and George Smalley; her friends, Rhody Peterson of La Porte and Bonnie Smith (Mark) of Fish Lake; and her pets, Indy and Shadow. She is also survived by numerous nieces and nephews.
