She was predeceased by: her parents, Clifford Raymond and Margaret Lucille Freese (Gallo); her children, Saundra Ann Dickie and Michael Lee Gurrola; and her brother Raymond Allen Freese.
She is survived by: her spouse Dale V. Applegate D. C.; her sons, Rudolph R. Gurrola, Jr. (Autumn) and Timothy J. Gurrola D. C. (Michelle) of La Porte, Indiana; her stepdaughters, Kari Sculati of Elkhart and Kristi Applegate of La Porte, Indiana; and her brother William R. Freese (Barbara) of La Porte, Indiana. She is also survived by 17 grandchildren; 10 great-grandchildren.
