He is survived by: his mother Ethel Koontz (Tidwell); his significant other Cassie Schultz; his children, Dustin Harvey, Leslie Koontz, Destiny Denham and Nicole Lynch; his grandchildren, Keith, Mylo Harvey, Thomas, Travis and Mason Schimmel; and his brother Danny Koontz.
