She was predeceased by: her parents, Charles Smith and Mattie Smith (Krueger); her husband Richard D. Cook; and her siblings, Madeline Willoughby, Charles R. Smith and Eugene L. Smith.
She is survived by: her daughters, Julie Ann Jongkind of La Porte and Judith Lynn Pahs of Sun Prairie, Wisconsin. She is also survived by eight grandchildren; 19 great-grandchildren; and several nieces and nephews.
