She is survived by: her children, Garry Polakovic (Sara) of LaOtto and Jeannie Polakovic (Jill) of Winamac; her grandchildren, Holly Smith (Clancy), Errik Polakovic (Toni), Emily Kraus (Nick) and Noah Polakovic (Chelsey); her siblings, Beatrice Civitavese of North Carolina and Alvin Chapman (Sandy) of Florida; and her sister-in-law Nancy Chapman of Ohio. She is also survived by 13 great-grandchildren.
