She was predeceased by: her mother Yvonne Perez. She is survived by: her father David Hart (Tammy) of Crown Point; her siblings, Adam Hart, Claire Hart, Andrew Winters, Alex Elkins, Adam Elkins, Andrew Elkins and Amanda Elkins; and her grandparents, Gary, Jean Hart, Danny, Betsy Matthews, Melva Winters and Patricia Holland. She is also survived by many aunts, uncles and cousins.

Memorial donations may be given to the family.
