He was predeceased by: his father James Shephard; and his father-in-law Darryl Rash. He is survived by: his wife Dawn Mattingly; his sons, John Lee Mattingly, Jr. (Brittany) and Dustin Michael Mattingly (Desiree); his mother Sharon Shephard; his mother-in-law Patricia Rash; his sister Paula Crews (Derek); his nephew Darrien Turner; and his grandchildren, Ryleigh Mattingly, Ryden Mattingly and Emma Mattingly.
