He was predeceased by: his parents, Albert I. Hazen and Betty Gerbofsky Hazen; and his siblings, Morty Hazen and Terry Ward. He is survived by: his daughter Brandi Miller (Eric); his stepdaughters, Terry Hutt (Bob) and Tammy LeRoy; his grandchildren, Makenzie and Tanner Miller; and his nieces and nephews, Marla, Nicole, Kelly and David.
A graveside service was held on Sunday, November 17th 2024 at 11:00 AM at the Etz Chaim Cemetery (2200 S West St, Indianapolis, IN 46225).
