He is survived by: his cousins, Larry Clark (Nicole) of Lebanon, IN and Dale Clark (Rita) of Plainfield, IN; his caregivers, Tammy Kelley, Sandy Vasquez, Marisa Vasquez and Jasmine Wilson; and his friend Paul Chiki.
