He is survived by: his mother Carol Uhl Phillips DeVary of Charlestown, IN; his wife Jessica Phillips of Henryville; his children, Zachary Phillips of Henryville, Ethan Phillips of Henryville and Katelyn Phillips of Henryville; and his brother Kevin Phillips of Charlestown.
