It is with deep sorrow that we announce the death of Diann C. Ptacek of Hebron, Indiana, who passed away on October 26, 2021, at the age of 80, leaving to mourn family and friends. You can send your sympathy in the guestbook provided and share it with the family.

She was predeceased by: her parents, Louis Jacobs and Audrey Jacobs; and her brother David Jacobs. She is survived by: her husband Leonard O. Ptacek; her brother Dennis Jacobs (Dyanna); and her friend Claudette Boyd. She is also survived by numerous nieces, nephews, relatives and many friends.

In lieu of flowers, memorial donations may be made to the family.
