He was predeceased by: his father Bradford Lawrence Church; his grandparents, Lou, Ruth Church and Berta Hynes; and his great-grandparent Helen Prince.
He is survived by: his mother Sharon Ann Bird Church of Louisville, Kentucky; his brother David Bird; his nieces and nephews, Atticus, Brad, Emmitt, Kyla and Natalie; his uncles, Daniel Beatty of Evansville and Matthew Church of Florida; and his aunt Twalla Long (Jesse) of Milton, Kentucky. He is also survived by several cousins and other relatives.
Memorial contributions may be made to the Logan Church Remembrance Fund.
