He is survived by: his wife Dawna; his children, Misty Theodore (Brent), Malisa Chavez and Mark Chavez (Kara); and his grandchildren, Zachary, Samantha (Will), Ashland (Dylan), Nick, Amber, Alexis and Eric. He is also survived by great grandchildren.
