She is survived by: her husband Matt Berkemeier; her children, Tanner Thompson, Ava Thompson, Easton Berkemeier, Cy Berkemeier and Joey Berkemeier; her granddaughter Blakely Thompson; her brother Bryce Mitchem (Melissa); and her parents, LeighAnn Mitchem and Carolyn Burkhart.
The family requests that any memorial contributions be made to the American Cancer Society in Jade's name.
