He is survived by: his wife Jean Hendren Polk (Babydoll) of Greenfield, Indiana; his children, Eddy Polk (Wendy) of Greenfield, Indiana, Susy Nickel (Ryan) of New Palestine, Penny Polk of Greenfield and Samantha Beauchat (Blake) of Greenfield, Indiana; his grandchildren, Brandon Radford (Samantha) of New Castle, Cody Polk (Andi) of Wilmington, Whitney Jefford of Greenfield, Matthew Polk of Greenfield, Zeke Jefford of New Palestine, Layni, Addison and Paisley Beauchat of Greenfield, Indiana; his great grandchildren, Eva, Hudson Radford of New Castle and Kade Jefford of Greenfield, Indiana; and his brothers, Charles Calhoun (Eddi) of Arkansas and Robert Polk (Roxane) of Knightstown, Indiana..
