He was predeceased by: his parents, Norma Blake (Hoover) and Robert D. Blake; his brother Robert A. Blake; and his granddaughter Avary Blake. He is survived by: his wife Jan Blake of Gas City; his children, Amos Blake (Amanda) of Jonesboro, Samantha Blake of Gas City and Whitney Blake (Steve Ott) of Marion; his stepdaughters, Chelsea Harris of Scottsdale, AZ and Courtney Charles (Frank) of Annapolis, MD; his grandchildren, Brody Blake, Lilah Greer, Arabella Blake, Maci Nicholson and Leo Charles; and his sisters, Shirley Hunt (Don) of Fort Wayne, Marsha Wallace (Howard) of Galesburg, MI and Becky Blake of Seattle, WA. He is also survived by many nieces and nephews.
