He is survived by: his sons, Daniel Ballard (Tasha) and Charles Ballard (Jadaha); his grandchildren, Cloe Ballard, Kiarra Ballard, Benjamin Eli Ballard and Anthem Draper; his great grandchildren, Asher of Lebanon and Bexley Gregor of Lebanon; his sister Tammi Anzalone (David) of Cedar Creek, NE.; and his former wife Melody Ballard.
