He was predeceased by: his parents, William S. Ellsworth and Maxine S. Ellsworth; and his sister Joanne Jummonville. He is survived by: his wife Sue S. Ellsworth of Fort Wayne, IN; his children, Kipp S. Ellsworth of Tucker, GA, Tami Anderson (Chris) of Alpheretta, GA and Tracey Champion (Brad) of Elkhart, IN; and his grandchildren, Brandon Anderson, Brooke Anderson, Nathan Sanderson and Abby Wilber.

In lieu of flowers, memorial donations in his honor may be directed to Lewy Body Dementia Society Research.
