He was predeceased by: his parents, Lewis II and Artis Evans II (Harnischffeger); his wife Sue Evans; his daughters, Lisa Duncan, Cindy and Connie Green; and his sister Virginia Graham (Gary Evans).

He is survived by: his sons, Daniel Evans (Deb), Butch, Ricky, Bobby and David Green; and his brothers, David and Charles Evans. He is also survived by 14 grandchildren; 23 great-grandchildren.

Contributions in Lewis's memory may be made to American Cancer Society.
