He was predeceased by: his parents, William Sullivan and Katherine Sullivan (Burns); and his brother Bill Sullivan. He is survived by: his wife Jaymi; his children, Jill Hite (Fred), John Sullivan, Jr. Amy (Cheryl), Elizabeth Palmer and Hayli Beck (Scott); his grandchildren, Katie Schooley (Jake), Emily Whitlock (Kyle), Gray Hite, Maggie Sullivan, Ryan Johnson, Pete Sullivan, Jack Hite, Sam Sullivan, Marie Johnson, Morgan Palmer, Joe Sullivan, Jude Johnson and Claire; his great grandchild Oliver Schooley; and his siblings, MaryLou Irick (Jay), Frank Sullivan, Steve Sullivan (Kathleen), Kathy Miner and Kris Rumschlag (Joe).
