With heavy hearts, we announce the death of Jack L. Fielitz Sr. (Fort Wayne, Indiana), who passed away on May 7, 2021 at the age of 79. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He is survived by: his sons, Jack L. Fielitz Jr. (Tammy) of Fort Wayne and Dennis D. Fielitz of Fort Wayne; and his brother Jerry D. "Jake" Fielitz of Harlan. He is also survived by three grandchildren; and eight great-grandchildren.

Memorial donations in Jack's honor may be directed to Northeastern Volunteer Fire Department, or Shriner's Hospitals for Children.
