It is with great sadness that we announce the death of Chad Jason Cook of Fort Wayne, Indiana, who passed away on July 6, 2023, at the age of 49, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He was predeceased by: his brothers, Darlo Cook, Tony Cook and Mark Cook. He is survived by: his parents, Carol Cook and Poff; his daughter Victoria Lynn Thomas; and his siblings, Joy Miller (Dave) of Ligonier, Cindy Anderson (Daniel Alemu) of Ligonier, Dawn Gibson of Ligonier, Shane Cook and Willy Cook of Fort Wayne. He is also survived by several nieces and nephews.
