He is survived by: his father Curt Sampson (Theresa) of Fort Wayne, IN; his children, Candice Hooks (JT) of South Bend, IN and Chad Sampson of Fort Wayne, IN; his siblings, Craig Sampson of New Haven, IN, Brian Sampson and Amber Ruiz (James) of Columbia City, IN; his stepsiblings, Keli Emeniser (Rex) of Monroeville, IN and John Mertz; and his grandchildren, Gabriel and Sidney of South Bend, IN..

In lieu of flowers, memorial donations in his honor may be directed to the family in care of Curt Sampson.
