She was predeceased by: her parents, William James and Margaret Barron James; her husband Frederick J. Koopman; and her son Frederick Koopman. She is survived by: her children, James W. Koopman, Theresa Bault (Mark), Mary Hentz (Anthony), Eva Perkins (Anthony) and Margaret Sobal (Larry). She is also survived by 15 grandchildren; and 20 great-grandchildren.

The family requests that contributions in Hazel's memory be made to the church.
