He was predeceased by: his father Jerrold Horn. He is survived by: his mother Beverly Horn (Tuley); his children, Joshua Allyn Horn (Atlanta) and Jacqueline Fulkerson (Phillip); his wife Kim Wohlgamuth-Horn; his grandchildren, Avery, Kada, Coralyn, Oakley, Elexis, Talonica and Jacelyn; his siblings, Denise King and Jeryl Hope (Patrick); and his nieces and nephews, Travis Enlow (Ka'Sandra), Brandee Powers (David), Jeremiah Hope (Allyce) and Jerrod Hope. He is also survived by several great-grandchildren, great-nieces and nephews.
Memorial contributions may be made in honor of Dale to Evansville Lutheran School and St. Paul's Lutheran Church.
