He was predeceased by: his parents, Thomas J. Koontz, Sr. and Sharon (Earl Wernes). He is survived by: his wife Janet (Bazant); his stepchildren, Jill (Anthony Carta) and Jason (Maria Spaw); his daughters, Jessica (Scott Morgan), Heather (Brady Hare) and Nicole Koontz (Zack Palmer); his grandchildren, Eleanore, Tyler, Gavin and Mila; and his sister Elizabeth Koontz (Ryanne Lozano).
